Anne Arundel nonprofits partner to form Chesapeake Health Partners

Hospice of the Chesapeake announced it has joined forces with Anne Arundel County nonprofit Partners In Care of Maryland Inc. under a newly formed umbrella organization, Chesapeake Health Partners.
This collaboration marks what Hospice of the Chesapeake called a “significant milestone” in both organizations’ shared mission to serve the state’s elderly population.
Under the new Chesapeake Health Partners, Hospice of the Chesapeake and Partners In Care will combine resources to expand their collective reach and impact across the region in an effort to amplify each organization’s ability to serve, according to a release.
By 2030, nearly one in four Marylanders will be 65 or older, according to Hospice of the Chesapeake.
“That’s not just a statistic, it’s a call to action,” Hospice of the Chesapeake President and CEO Becky Miller said.
“By coming together, we can ensure that older adults in our community continue to receive the care, support and connection they need to thrive.”
Hospice of the Chesapeake, based in Pasadena, is Maryland’s largest independent nonprofit hospice organization specializing in providing care for families facing serious illness. Partners In Care, also based in Pasadena, is an organization priding itself on aiding older adults remain independent and engaged through its Service Exchange model.
The model involves Partners in Care members assisting each other with transportation, handyman services, social connection and more, a release says. Hospice of the Chesapeake also develops care plans to minimize symptoms for patients and families in Anne Arundel, Calvert, Charles and Prince George’s counties.
“Together, we can build a stronger safety net for older adults, one that keeps people in their homes, connected to their neighbors and supported through every stage of aging and illness,” said Brian Hahn, Partners In Care of Maryland’s board of directors chair.
Despite the new umbrella organization, Partners In Care “will retain its name, mission and volunteer-driven programs, ensuring continuity for members, volunteers and staff” as the nonprofit aims to play a vital role in enabling older adults to remain active and independent.
